NDIS invoice template

An NDIS invoice needs your business name and ABN, the participant's name and NDIS number, and for each support the support item number, date, quantity, price and claim type, plus the total and any GST. One invoice covers one participant.

What the NDIS asks for on an invoice

The NDIS lists these details for provider invoices (ndis.gov.au, Invoicing and record keeping, checked 30 October 2026):

FieldExample
Your business name (as registered with your ABN)Sam Taylor Support
Your ABN12 345 678 901
Participant's name and NDIS numberJordan Lee, 430 123 456
Participant's addressOnly for specialist disability accommodation
Support item number from the NDIS Support Catalogue01_011_0107_1_1
Quantity and price for each support3 hours at $65.00
Claim typeStandard, cancellation, provider travel…
Date or dates the support was delivered20/10/2026
Total invoice amount$195.00
GST, if applicableMost supports are GST-free

An invoice can only be for one participant, but it can include several supports. Plan managers who pass your invoice on also add your ABN as the third-party provider, which is why a correct ABN matters.

Details plan managers also expect

  • Invoice number and date, so payments can be matched.
  • Your bank details (account name, BSB, account number) and payment terms.
  • The plan manager's name on the “bill to” line for plan-managed participants.
  • Start and finish times on hourly supports: not on the NDIS list, but they make it easy to check the weekday, evening, Saturday or Sunday item.

Common reasons an invoice comes back

  1. The rate is above the price limit for the support item in your state or territory.
  2. A weekday item is used for a Saturday, Sunday or evening shift.
  3. The support item number has a typo or is from an old price guide.
  4. The ABN or NDIS number is wrong, or two participants are on one invoice.
  5. The support date is outside the participant's plan dates.

The generator checks the first four for you. Price limits come from the 2026-27 NDIS Support Catalogue; see support item numbers.

Questions

Do NDIS invoices need GST?

Most NDIS supports are GST-free, so the invoice shows no GST and is not a tax invoice. If a line is taxable and you are registered for GST, show the GST amount and title the document “Tax invoice”.

Can I put several participants on one invoice?

No. The NDIS says an invoice can only be for one participant. It can list many shifts and supports for that participant.

Do unregistered providers use the same invoice?

Yes. Unregistered providers can support plan-managed and self-managed participants, and the same details apply. NDIA-managed participants can only use registered providers.

Is there an official NDIS invoice template?

The NDIS lists what an invoice should contain but does not require a particular layout. Any invoice with those details works.
